NOMINATION FORM: 2013 Wisconsin Music Therapist of the Year
MUSIC THEAPIST OF THE YEAR RULES AND REGULATIONS
Nominations for the “2013 Wisconsin Music Therapist of the Year Award” may originate from any source including a friend, colleague, employer, family member, client, client family member, etc. Any music therapist in the state of Wisconsin may be nominated, regardless of membership status in the Wisconsin Chapter for Music Therapy (WCMT). A music therapist may NOT nominate him/herself for this award.
The nominated therapist will be notified of the nomination. The winner will be chosen by visitors to the WCMT website who cast their vote via a link from a dedicated nominee biography webpage. Anyone visiting the website from January 1st through the 31st, 2013 may vote for the therapist they would like to receive the award, but votes are limited to one per IP address. The therapist with the highest number of votes will win the award of “2013 Wisconsin Music Therapist of the Year” and receive a $100 gift certificate for West Music products in addition to a free CMTE workshop or class provided by WCMT. In addition, the recipient of the award will be featured in an article released during music therapy month in Wisconsin in March, 2013.
